Foundation for Community Health (“FCH,” “we,” “us,” or “our”) is a health foundation serving the 17-town region where northwest Connecticut meets New York’s Greater Harlem Valley. We invest in community-led solutions to improve the health of residents in our region through grantmaking and advocacy.
